One of the most frustrating things patients tell us is:

“I already had treatment for my gums. Why has it come back again?”

It is a very understandable question — and in many ways, gum disease can behave a little like other chronic inflammatory conditions in the body. It can improve significantly, remain stable for years, and then flare up again if the balance changes.

Periodontal disease is not simply “dirty teeth”

What many people do not realise is that periodontal disease is not simply “dirty teeth” or something solved with one deep clean. It is a long-term condition influenced by many different factors: the individual immune response, genetics, stress, smoking or vaping, hormonal changes, medical conditions, grinding, mouth breathing, difficult anatomy around the teeth, and of course daily maintenance over time.

In practice, we often see patients who had periodontal treatment years ago and remained comfortable for a long time, only to notice bleeding, inflammation or bone loss returning later in life. Sometimes nothing dramatic has changed. Sometimes life simply becomes busy, maintenance appointments become irregular, or the body reacts differently with age.

Maintenance is one of the most important parts

Another common misunderstanding is the belief that once gums have been treated successfully, they no longer require ongoing professional care. In reality, maintenance is one of the most important parts of periodontal therapy. Even patients with excellent home care may have areas that are difficult to clean consistently over many years.

While newer diagnostic tests and bacterial analysis are increasingly discussed online, gum disease is still primarily diagnosed and managed through careful clinical assessment, radiographs, long-term monitoring and maintenance. In many cases, the fundamentals remain the most important: meticulous cleaning, reducing inflammation, stabilising the bone and supporting patients over time.

Successful care is rarely about one single treatment

At Implant & Perio Clinic in Marylebone, near Harley Street, we often explain to patients that successful periodontal care is rarely about one single treatment. It is usually about building long-term stability and understanding how the condition behaves over the years.

The good news is that recurrent gum disease does not necessarily mean failure. Very often, when identified early, it can be stabilised again successfully with the right professional care and maintenance approach.
